Forms on this page include the Practicum Application, forms specific to the Health Informatics Concentration, and the Practicum/Field Experience Request for Waiver.
One exception is for students who are also pursuing a School Library Certification. School Library Certification requires INFO 5090.993/94. It includes a Proficiency Checklist and Mentor as assigned at the beginning of the certification coursework. School Library Certification students should go to the Practicum Requirements page for information on how to fulfill their specific practicum requirements.
